Four steps, always with the case manager at the helm: a home assessment, a care plan, team coordination and a report every week.
Assessment at home
The case manager comes to your home. They assess the person and the environment using validated clinical indices, listen to the family, and return a report and a proposed plan, credited against your first month if you go ahead.
Care plan
Together we define what is needed, how often and by whom: case manager visits, clinical procedures, therapies, care technician hours. Everything in writing, every price known upfront.
Coordination
The team comes into the home and the case manager coordinates: scheduling, liaison with the hospital, adjustments to the plan, response to complications. The family stops making phone calls.
Weekly report
Every Friday, by email: what happened, how things are evolving, and what comes next week. In clear language.
